INDIVIDUAL PLANNING
Consumers participate in the development of an Individual Plan of Care (IPC) which assists in the understanding of the services that are being sought and identifies expectations for the coming year. This plan can be changed as needed but will be reviewed at least once each year. Consumers are expected to take an active role in the development of the IPC including sharing strengths, needs, wants and service expectations. A case manager or clinician is available to assist in the service planning process to ensure that it is clearly focused on the client's goals. Consumers may choose to have others, such as family, involved in the treatment planning.
EXPERIENCED STAFF
Rutland Mental Health Services is highly committed to supporting consumers with the quality services they need. Our dedicated staff of caring professionals has been trained to meet federal, state and other accreditation requirements for professional development. In addition, all staff members are expected to abide by the Code of Conduct established by Rutland Mental Health Services. A copy of this Code is available upon request.

At Rutland Mental Health Services, we strive to provide highly individualized, quality services to each person we assist. As part of our ongoing efforts to broaden and enhance our services, we ask our consumers for input. Our consumers and/or their guardians are always encouraged to provide feedback to our staff at any time about their experiences with Rutland Mental Health Services. In addition, at least once a year, we ask our consumers and/or their guardians to complete a questionnaire to assess their satisfaction with our services.

SERVICES
Community Rehabilitation and Treatment Program
Adult Case Management
Assists individuals in developing and attaining goals, in accessing community resources, and solving problems.
Outpatient Treatment
Provides individual and group therapy to allow individuals to develop improved coping strategies.
Medication Management
Psychiatric evaluation; information regarding psychoactive medications; and medication management.
Transitional Living
Temporary housing for individuals that would benefit from a supported living situation while in transition from a higher level of care, or as an alternative to a higher level of care.
Housing Assistance
Assistance in locating and obtaining housing, including apartments and residential care settings; assistance in obtaining housing subsidies.
Co-occurring Disorders
Comprehensive, integrated services for individuals with co-occurring mental health and substance use problems.
VOW - Vocational Opportunity Works
Provides an array of supported employment services to individuals with mental health and co-occurring disorders.
EMERGENCY SERVICES
Professional staff available 24 hours per day, 7 days per week for phone or face-to-face intervention to assist members of the community in developing and achieving a safety plan in crisis situations. Call 802-775-1000, 24-hour emergency/crisis line.
ELDERCARE
Provides outreach mental health services to elders, including assessment and case management.
EVERGREEN SUBSTANCE ABUSE SERVICES
Provides adult substance abuse treatment services, including Assessment and Referral, and Quitting Time, an intensive outpatient substance abuse treatment program. Continuing Care is provided in the Aftercare Program. Community Services include the Rocking Horse Program for women of all ages including those with young children and pregnant women who are substance users or whose lives are affected by the substance use of a member of the household, Project CRASH (state-sponsored), and Drug Court services.
