• To encourage and support each individual to identify and attain his/her personal goals.
• To assist each individual with the development of meaningful and mutually supportive relationships with family and friends.
• To encourage each individual to make the choices and decisions that affect his/her life.
• To assist each individual to obtain employment or engage in other meaningful activities.
• To encourage each individual's active participation in the community and utilization of community resources.
• To assist each individual to live where he/she wants to live, and be part of the community in which her/she lives.

Our services cover a broad spectrum to effectively support the needs of the developmentally disabled residents of Rutland County, including:

• Child & Family Supports
• Career Choices Employment Services
• Community & Social Supports
• Nursing Services
After eligibility and state funding priorities are established, the services are tailored to the individual's needs.

INDIVIDUAL PLANNING
You will participate in the development of your Individual Support Agreement (ISA). This document will help us to understand the services that you seek and to identify your expectations for the coming year. This plan can be changed as needed but will be reviewed with you at least once a year.
You and your guardian are expected to take an active role in developing your ISA. This may include sharing your dreams, needs, wants, and service expectations with us. A Case Manager or other CAP representative will be available to assist in the service planning process to ensure that we are clearly focused on your goals.
EXPERIENCED STAFF
CAP is highly committed to supporting our developmentally disabled consumers with the quality services they need. Our dedicated staff of caring professionals has been trained to meet federal, state, and other accreditation requirements for professional development. In addition, all staff members are expected to abide by the Code of Conduct established by Rutland Mental Health Services; a copy of this Code is available at your request.
A PARTNERSHIP
At CAP, we strive to provide highly individualized, quality services to each person we assist. As part of our ongoing efforts to broaden and enhance our services, we ask our consumers for input. Our consumers and/or their guardians are always encouraged to provide feedback to our staff at any time about their experiences with CAP. In addition, at least once a year, we ask our consumers and/or their guardians to complete a questionnaire to assess their satisfaction with our services.
