Developmental Disabilities

The Community Access Program (CAP) was created to help residents of Rutland County with developmental disabilities to improve the quality of their lives. We support them and their families to reach their potential, by identifying personal goals and empowering them to achieve those goals. We believe that every individual has the right to live in and be a valued, respected member of the community. Our goals are based on the need to work with families and other service providers to develop the supports necessary within the community to assist individuals with developmental disabilities to identify and achieve personal goals. By doing so, we empower individuals to grow, and gain greater independence while increasing their self esteem in the process.

OBJECTIVES

  • To encourage and support each individual to identify and attain his/her personal goals.
  • To assist each individual with the development of meaningful and mutually supportive relationships with family and friends.
  • To encourage each individual to make the choices and decisions that affect his/her life.
  • To assist each individual to obtain employment or engage in other meaningful activities.
  • To encourage each individual's active participation in the community and utilization of community resources.
  • To assist each individual to live where he/she wants to live, and be part of the community in which her/she lives.

SERVICES

Our services cover a broad spectrum to effectively support the needs of the developmentally disabled residents of Rutland County, including:

  • Residential Services
  • Case Management
  • Child & Family Supports
  • Career Choices Employment Services
  • Community & Social Supports
  • Nursing Services

After eligibility and state funding priorities are established, the services are tailored to the individual's needs.

INDIVIDUAL PLANNING

You will participate in the development of your Individual Support Agreement (ISA). This document will help us to understand the services that you seek and to identify your expectations for the coming year. This plan can be changed as needed but will be reviewed with you at least once a year.

You and your guardian are expected to take an active role in developing your ISA. This may include sharing your dreams, needs, wants, and service expectations with us. A Case Manager or other CAP representative will be available to assist in the service planning process to ensure that we are clearly focused on your goals.
