The Community Rehabilitation and Treatment Program (CRT) supports individuals through this process with a wide range of services.

ELDERCARE

Older adults can be at risk for mental health problems, impacting their quality of life and making existing health problems difficult to manage. Eldercare support at Community Care Network provides outreach mental health services to elders, including assessment and case management.

We aim to ensure older adults can access the help they need for mental and emotional distress. Additionally, we offer in-home substance use disorder counseling and caregiver support.
